I have hereby consent to the procedure of obtaining blood by an appropriately train phlebotomist for BLOOD-LAB LTD. This procedure has been explained to me by the phlebotomist and I understand the nature of the procedure is the removal of my blood. Blood-Lab LTD is obliged to provide test results using third party contractors, but not Take any responsibility for the contain of test results, and do not provide any interpretation of the results. I understand that the material risks of this procedure or treatments may include but are not limited to: risk of infection or bleeding, swoon, allergic reaction, nausea, vomiting, lowered blood cell count which may cause anaemia, vain irritation, dizziness the decreased blood pressure, diaphoresis (clammy skin), pain or bleeding at site, redness, fatigue, cardiac arrest, and other which can result in life threatening changes. Blood-Lab LTD reserves the right to ask the patients to re-submit samples in case of collected samples have been damaged destroyed contaminated or affected by other factors that prevent the test being carried out.
